Birth and Death Certificates

Certified copies of birth and death certificates may be obtained at the West Allis Health Department for births or deaths that occurred in a West Allis residence or a West Allis health care facility: West Allis Memorial Hospital or a nursing home in West Allis. The charge for a birth certificate is $20.00. Additional copies ordered at the same time are $3.00 each. The charge for a death certificate is $20.00.
